• Focus Area: Early detection, prevention, and ongoing management of non-communicable diseases (NCDs), including diabetes screening, hypertension control, and obesity awareness in India.
• Key Activities: Organize 12 NCD health camps, conduct 24 preventive health workshops, implement digital tracking for beneficiaries, distribute 100 essential health kits, and train 50 local health workers for sustainable NCD care.
• Beneficiaries: 800–1,000 direct participants and approximately 2,500 indirect beneficiaries, with priority for women, SC/ST/OBC groups, and low-income families in rural Indian communities.
• Expected Outcomes: Achieve a 30% reduction in undiagnosed NCD cases and a 60% boost in community health awareness, promoting long-term preventive care in India.
• Sustainability: Ensure ongoing impact through trained ASHA/ANM workers, linkages with primary health centers (PHCs), and community-led health committees for self-sustaining NCD prevention programs.

Comprehensive proposal including objectives, activities, timelines, budget, sustainability plan, Gantt Chart, Organogram of team structure, a logical framework (Logframe), detailed budget, a sustainability plan, an exit strategy, and anticipated benefits to HPCL, impact assessment details and a separate folder containing images..
